  1. You must have purchased a PIN from any ZENITH BANK BRANCH, this pill will allow you access to fill the form.
  2. You must complete the form with accurate data, falsifying information or impersonation is a federal offence and is punishable by law.
  3. On completing the form please send the following to the email address provided below:
    • Close up photo of your face (head shot photo)
    • Full length swim suit photo (one or two piece, no shorts!)
    • A full length fashionable photo (formal, casual or short dress)
    • Each photo must be titled with your full name
    • Your full name must be the subject of the email.
  4. Each photo must not be more than 500KB and should preferrably be 5" X 7"
